Dr. Pan Xincheng said that the effect of allogenic skin covering other people's skin is better than artificial skin or pigskin. (Photo/Chengda Hospital) Pan Xincheng, a plastic surgeon at Chengda Hospital, said that in general, the principle of wound management for severely burned patients includes entering the wound within one week. Debridement, progressive wound? Autologous skin transplantation, and temporary covering of wounds without skin graft with dressing. Patients with severe burns and scalds are limited by limited donor sites. Autologous skin transplantation often requires multiple operations, and sometimes the skin can be taken again after the donor site heals, which often delays the time of wound coverage. Some patients are temporarily covered with biological or synthetic dressing, which has the advantages of reducing bacterial infection and promoting wound regeneration.
In addition to autologous skin transplantation, there are three common dressings: artificial skin, heterogeneous skin (pig skin) and allogenic skin (other people's skin). Artificial leather does not need to be stored in leather warehouse, but it is expensive and the result is often embarrassing; The adhesion of allogenic skin is better than artificial skin, but its biology is not as good as allogenic skin. However, allogenic skin has the highest compatibility with patients' own skin tissue, lower infection risk and better wound healing effect, and is most suitable for covering burns and scalds, but its source is limited.
At present, the source of allogenic skin is mainly the donation of corpse tissue. According to the statistics of organ donation last year, 229 donors donated 877 organs, of which only 30 were skin donations. According to analysis, the reasons for the low proportion of skin donation are: first, family members want to keep the general appearance intact, and second, they are worried that improper treatment of wounds will cause regret.
However, Dr. Pan Xincheng explained that according to experience, skin removal is limited to trunk, back and limbs, and exposed parts of clothes, such as head and neck, hands and feet, remain intact. The harvesting process is carried out by a special machine, and the skin is about 0.3 mm thick and as thin as paper, so it will not affect the general appearance. In addition, the skin is wrapped with clean gauze after the internal organs are harvested, so there will be no bleeding from the wound. Therefore, people are urged to give their love and skin to help burn patients.
