People with mild deformity can wear braces to strengthen back muscle exercise. The flexibility of spine bending can be measured by taking X-rays in traction state and natural state. During wearing the bracket, the angles of original bending and compensation bending should be recorded so as to monitor their development. If the curvature is obviously aggravated during the fixation of the brace, surgical treatment should be considered. If the curvature exceeds 50, it is best not to fix it with braces.
2. Surgical therapy
According to the type and severity of spinal deformity, the progress speed of scoliosis, the location of deformity and the age of pathogenic bacteria, the surgical method is determined.
(1) Posterior spinal fusion in situ is suitable for patients with mild or moderate spinal deformity, acceptable appearance and slow development of deformity, especially those with unilateral unsegmentation. Autologous iliac bone can be used as bone source, and the fusion range includes upper and lower normal vertebral bodies.
(2) The unilateral vertebral epiphysis was fixed and fused before and after the convex side of the vertebral body to prevent it from overgrowth, so that the concave side of the spinal column could continue to grow and achieve the purpose of orthopedic.
(3) Scoliosis correction and fusion is suitable for patients with severe spinal deformity. Spinal traction is needed before operation to prevent spinal cord injury caused by sudden elongation of the spine during operation.
(4) Hemivertectomy is suitable for spinal tilt and decompensation caused by sacral junction. After the hemivertebra was taken out, it was fixed with a compression rod. If there is spinal bifurcation or neurological insufficiency, semi-fusion between convex lumbar vertebrae and sacral vertebrae is needed.
(5) Spinal osteotomy is suitable for elderly or adolescent patients with unilateral unsegmented vertebral body, stiff lesion and severe angulated scoliosis. If there is rib fusion on the axial side of the spine, it should be removed during the operation.
