What is cleft lip and nose deformity?
The repair of cleft lip nose deformity is a complicated work. Although there are many methods at present, it is still difficult to achieve ideal reduction effect. This is determined by the characteristics of cleft lip and nasal deformity. The treatment of cleft lip nose deformity has also been a hot topic for scholars. To repair cleft lip nose, we must first understand the characteristics of cleft lip nose deformity. The severity of cleft lip nose deformity varies from person to person. The clinical characteristics of cleft lip nasal deformity are simple asymmetric nasal deformity, which can be roughly summarized as follows: (1) the tip of the nose is low, inclined to the healthy side or split; (2) flat collapse of the alar, abduction and drooping of the alar, and collapse of the nasal vault; (3) The nasal columella on the cleft side is shortened, the nasal columella is deviated, and in severe cases, the nasal dorsum is deviated; (4) When the nostril is placed horizontally, the base of the nostril is asymmetrically widened or sunken (too large or too small); (5) Vestibular fold of nostril; (6) The base of nostril collapses, and the platform of nostril base is defective or has a small gap. The surgical methods of cleft lip nose deformity are different according to the different characteristics of cleft lip nose. The plastic surgery department of Shanghai Armed Police Hospital used island flap and cartilage transplantation to repair deformity. Methods: 1) according to the height of the columella, island flaps were formed at the base of the columella and on both sides of the alar, and fully dissociated to the tip of the nose; Separate alar cartilage on both sides to expose columella cartilage; 2) Raise the flap and alar cartilage together, and fix alar cartilage on the healthy side of the nasal tip; 3) Take the patient's own costal cartilage, cut it into thin slices, and insert it between the alar cartilage on both sides for fixation; 4) Finally, pull down the island flap, cover the wound and suture, and if necessary, perform Z-plasty for nasal contracture. Island skin flap and cartilage transplantation can improve the nasal tip, lengthen the columella, enlarge or narrow the nostrils, thus improving the nasal appearance. Plastic experts of Shanghai Armed Police pointed out that this method must make the alar and cartilage fully separated and the tissue completely removed. The adaptive age of this operation is more suitable for teenagers. In addition, considering that the postoperative recurrence of nasal deformity is mostly caused by human growth and development, parents of children should take them to the hospital for treatment at an appropriate time.
