What materials do clinics need to submit for filing?
According to the Interim Measures for the Administration of Clinic Filing (Guo Wei Zheng Yi Fa [2022] No.33)

Article 4:

Units or individuals that set up clinics shall file with the health administrative department of the people's government at the county level or the competent department of traditional Chinese medicine where the clinics are to be set up, and only after obtaining the Clinic Registration Certificate can they carry out their practice activities.

Article 6:

The clinic shall submit the following materials for the record:

1. Outpatient record information table;

2 outpatient room plan (referring to the outpatient room marked in proportion, indicating the functional distribution and area size);

3. Certificate of property right or lease contract of clinic house;

4. A copy of the valid identity certificate, relevant qualification certificate and practice certificate of the legal representative and principal responsible person of the clinic;

5. Directory of other health technical personnel, valid identity certificates and copies of relevant qualification certificates and practice certificates;

6. Outpatient rules and regulations;

7. List of clinical instruments and equipment;

8 attached to the pharmacy (cabinet) drug list;

9 clinic sewage, dirt, feces treatment plan and the surrounding environment of the clinic;

10. Other relevant materials required by laws and regulations.

Where a legal person or other organization establishes a clinic, it shall also provide the qualification certificate of the legal person or other organization, the identity certificate of the legal representative or the identity certificate of the representative of other organization.
