For the treatment of elephantiasis of penis and scrotum, anti-filariasis drugs such as oral ethambutol should be given first. If there are signs of skin damage and bacterial infection, mupirocin cream should be used externally to prevent further infection. Severe scrotal elephantiasis can be treated by surgery, and hyperplasia and edema tissues need to be removed for skin transplantation. Restore the appearance and function of genitals as much as possible.

First, medication.

Generally, the corresponding drugs are selected according to the pathogen, and the penis and scrotum elephantiasis caused by pathogen infection can be solved from the root. Pyrimethamine is the first choice for elephantiasis caused by filariasis. If there are signs of damage and bacterial infection on the skin surface, mupirocin cream should be used externally to prevent further infection.

Second, surgical treatment.

Severe scrotal elephantiasis can be treated by surgery, and the specific surgical methods are divided into two categories.

1. Extensive resection and extra shaping of diseased tissues are classic and widely used methods. The main method is to completely remove the whole diseased tissue to the spermatic cord capsule according to the requirements of plastic surgery, and then reconstruct the scrotum and/or penis with the remaining healthy peripheral skin flap or pedicled abdominal skin flap.

2. Lymphatic anastomosis was added on the basis of the above lesion resection. Additional lymphangiovascular anastomosis has been applied at home and abroad. After full-thickness resection of the diseased tissue, the expanded lymphatic vessels in the inguinal region are anatomized and anastomosed with the great saphenous vein or its branches, or with the blood vessels at the stump of lymph nodes after decapitation.
