Yi Xiaoling was shot through the face and neck in the Philippines and came to Taiwan Province to save face. The operation of Chang Gung Memorial Medical Team took 65,438+00 hours. Wei Fuquan said that this is the most difficult operation. (Photo courtesy/Chang Gung Hospital) According to the medical team's assessment, Yi Xiaoling's mandibular defect is 8 cm with displacement. The remaining bilateral mandibles were displaced inward and upward respectively due to the loss of support in the middle segment defect. In addition, the remaining right mandible has no teeth at all, which poses a reconstruction challenge to restore the normal occlusion of the upper and lower jaws. However, due to the original gunshot wound, repeated surgery, infection and other reasons, the oral cavity, chin and neck tissue have produced serious scar contracture, and the skin tissue is not easy to relax, so the operation is full of difficulties and challenges.
Surgery combines oral and maxillofacial surgery with plastic reconstruction surgery.
The operations were carried out by a team led by Dr. Cai, an oral and maxillofacial surgeon, and a medical team led by Wei Fuquan, director of the allogeneic composite transplantation center of Linkou Chang Gung Hospital. For the oral and maxillofacial region, firstly, the contracture scar of the face and neck was removed and the residual and severely displaced mandible on both sides was restored. Then the bone plate was reconstructed and fixed with titanium metal molded according to the model printed 3 days before operation.
Preoperative preparation includes printing out a three-dimensional model reconstructed by computerized tomography, which can be used to correct severe mandibular displacement and make titanium reconstruction bone plate. But also includes neck MRI, angiography, repeated consultation and discussion and division of labor planning.
