2. Take the form of the public security department to the cosmetic hospital and ask the hospital to issue a certificate that the applicant has received relevant cosmetic surgery according to the requirements of the form.
3. Then take the form of the household registration department and ask the street office (neighborhood Committee/village Committee) to issue the identity certificate of this person as required, and it will be effective if it is signed and sealed.
4. After the above procedures and processes are completed, take photos before and after the plastic surgery, take the information back to the household registration department, let them review, and fill in the relevant procedures and take photos to apply for a new certificate; If it fails to pass the audit, supplement the materials as required.
From the date of acceptance, you can get a new ID card within 60 days. In areas with inconvenient transportation, the processing time can be appropriately extended, but it can be collected within 90 days at the longest.
