In the 17th year of the Republic of China (1928), Yongkang County was established as Wudu County.

In the 18th year of the Republic of China (1929), it was renamed Kang County. Taking the meaning of "peace and well-being", the Northern Zhou Dynasty was named after Kangzhou.

In 2000, Kangxian administered 8 towns and 20 townships: Zuitai (later changed to Chengguan), Pingluo, Dabao, Anmenkou, Lianghe, Changba, Yangba, Yuntai, Wangguan, Gong Ji, Sitai, Dananyu, Miba, Sanguan, Wang Ba and Nianba.

June 20 15, 6 townships, namely Douping Township, Wang Guan Township, Wang Ba Township, Dananyu Township, Nianba Township and Ba Dou Township, were abolished and changed to towns, and Douping Township was changed to Zhoujiaba Town. There are eight ethnic groups in Kangxian, including Han, Hui, Tibetan, Manchu, Mongolian, Yao, Zhuang and Uygur.
