Examination and approval conditions:
(1) Food: conforming to the national hygiene standards for food industry, hygiene management measures and hygiene standards, food hygiene management standards and requirements for catering industry, and hygiene standards for students' nutritious meals production enterprises; After medical examination and health knowledge training, patients with infectious diseases are transferred from their jobs of directly serving customers.
(2) Drinking water: establish drinking water hygiene management rules and regulations, and equip full-time or part-time personnel to be responsible for drinking water hygiene management; Personnel directly engaged in water supply and management have passed the physical examination and health knowledge training; The drinking water supplied meets the national sanitary standard for drinking water; Centralized water supply units must have water purification and disinfection facilities and necessary water quality inspection instruments, equipment and personnel to carry out daily testing of water quality; Secondary water supply facilities shall also comply with GB17051-1997 Hygienic Standard for Secondary Water Supply Facilities.
③ Public places: meeting the corresponding hygiene standards and requirements of public places; Employees pass the physical examination, and patients with infectious diseases are transferred from their jobs that directly serve customers; The employee's health knowledge training is qualified.
Application materials:
① Food hygiene license:
Employee health examination certificate; Qualification certificate of health knowledge training for employees; Housing property certificate or lease certificate; Qualification certificate of legal person or legal representative; Health management organization and system; Basic sanitation facilities; Equipment layout and process flow chart; On-site acceptance report of municipal health supervision institutions (issued by municipal health supervision institutions); Preventive health supervision data (approval letter for design health review of new construction, reconstruction and expansion projects; Confirmation of health acceptance for the completion of new construction, renovation and expansion projects).
Food processing enterprises need to provide the following information:
Enterprise standards (filed by Technical Supervision Bureau); Enterprise laboratory information (inspection equipment, inspection items, name, gender, age, education) or entrusted inspection certificate; Two copies of samples, labels and instructions of finished packaged products; The recent inspection report of the inspection institution approved by the municipal public health administrative organ; Other materials required to be declared by the municipal health supervision institution.
(2) Hygiene license for public places:
Employee health examination certificate; Qualification certificate of health knowledge training for employees; Housing property certificate or lease certificate; Qualification certificate of legal person or legal representative; Health management organization and system; Basic sanitation facilities; The plan of the business place; The recent monitoring report of the inspection institution approved by the municipal public health administrative organ; On-site acceptance report of municipal health supervision institutions (issued by municipal health supervision institutions); Preventive health supervision data (approval letter for design health review of new construction, reconstruction and expansion projects; Confirmation of health acceptance for the completion of new construction, renovation and expansion projects); Other materials required to be declared by the municipal health supervision institution.
(3) Renewal or review of hygiene license:
Employee's physical examination certificate (copy); Certificate of health knowledge training for employees (copy); Qualification certificate of legal person or legal representative (copy); The recent monitoring and inspection report of the inspection institution approved by the municipal public health administrative organ; On-site acceptance report of municipal health supervision institutions (issued by municipal health supervision institutions); Original health permit; Other materials required to be declared by the municipal health supervision institution.
(4) Work permit for X-ray device:
Three-level protection responsibility system of the applicant's radiation protection institution (person in charge of the unit, person in charge of the protection institution and radiation workers); Radiation protection facilities and equipment (including buildings, interlocks, signs, protective articles and other materials); A copy of the radiation work permit or the certificate of training and physical examination for radiation workers; Safety operation procedures and radiation protection management system; Copy of X-ray equipment manual (medical X-ray machine does not report this item); Radiological health protection and quality control test report of medical X-ray machine; Other materials required to be declared by the health supervision institution.
Approval procedure:
① Notification: The health administrative department informs the applicant in writing of the Notice of Health Administrative License and its related laws, regulations, standards, codes of conduct and legal responsibilities in the window of the municipal administrative examination and approval service center. When issuing an application for health permit, the health supervisor requires the applicant to register at one time and inform the required materials.
(2) Commitment: The applicant shall provide materials as required by the notice, fill in the application form for health permit and sign it. The applicant shall promise to the administrative department of health to abide by laws, regulations and norms, and ensure that the information provided by him is accurate, true, legal and effective.
③ Application for registration: The window health supervisor of the municipal administrative examination and approval center will check the contents of the registration form filled in by the applicant and the information provided, and then register.
④ Acceptance: The Health Supervision Office of the Municipal Health Bureau examines the application materials for registration:
A. If the application is qualified and the information is complete, notify the applicant in writing to accept it.
