It is generally believed that helicobacter pylori infection and immune factors are the main factors. Drinking, improper diet, mental factors, drugs that can damage gastric mucosa (such as aspirin and indomethacin) and bile reflux are the inducing factors.

The inflammation of superficial gastritis is limited to the superficial layer of gastric pits and the lamina propria of mucosa. Epithelial cell degeneration, epithelial hyperplasia of pits and infiltration of lamina propria cells cause mucosal congestion, edema and exudation. Mostly in the antrum of stomach, sometimes accompanied by a small amount of erosion and bleeding. Some patients have more erosive lesions in the antrum of the stomach, or are accompanied by verrucous processes, which is called chronic erosive or verrucous gastritis.

Atrophic gastritis, inflammation goes deep into the lamina propria of mucosa, leading to atrophy of lamina propria, thinning of mucosa, flattening or even disappearance of mucosal folds, often accompanied by intestinal metaplasia and atypical hyperplasia.

The dysfunction of pyloric sphincter in some patients leads to the reflux of duodenal contents into the stomach, and bile reflux gastritis is caused by bile reflux.

Chronic gastritis is often complicated with gallstones or chronic ulcerative colitis.

Traditional Chinese medicine knowledge

Spleen and stomach weakness is its internal cause, and improper diet, emotional damage and excessive work and rest are its inducing factors.

Poor diet, stagnation, depression stagnation; Anger, liver failure, stomach and wrist qi stagnation; Overworking will consume qi and blood, and excessive comfort will lead to discomfort of qi.

Spleen and stomach weakness, combined with the above factors, leads to poor qi movement, stagnation of stomach qi and abnormal ascending and descending, which is the basic pathogenesis of this disease.

Symptom differentiation

Most chronic gastritis has no obvious symptoms, and some patients have symptoms such as fullness and discomfort in the upper abdomen, dull pain in the stomach, belching and noisy, anorexia and nausea. General gastritis can be seen anorexia, weight loss, or anemia; Gastric antrum gastritis or gallstones, especially those with more bile reflux, can cause acute erosion and obvious pain, murmur and burning sensation. In severe cases, a small amount of upper gastrointestinal bleeding may occur, such as hematemesis and melena.

Chronic gastritis belongs to the categories of stomachache, fullness, belching, noise and so on in Chinese medicine books.

After years of exploration, I have summarized the syndrome types of spleen and stomach weakness, stagnation of stomach qi and disharmony between liver and stomach, which may be combined with excess fire in liver and gallbladder and damp-heat in intestines and fu organs.
