Please fill in the front of the hospitalization card, the reverse side and the ID number and submit it to the hospitalization office. The hospitalization office will provide you with the registration and waiting procedures and issue a bed registration certificate, and then you will go home and wait for the notification of hospitalization.

For local patients whose hospitalization card states that they need to use blood, please go to the hospitalization office to get a blood notice, then go to the relevant unit to complete the blood application procedures, and then Go to the hospitalization office with the blood notification receipt and hospitalization card to complete the bed waiting procedures.

If you fail to receive a notification after registration, it means that the hospital bed has not been available yet, please continue to wait.

If the patient’s registered address and phone number change, please notify the hospital admissions office in a timely manner.

If your condition changes suddenly during the registration waiting period, you should be sent to the hospital for diagnosis and treatment at any time.

After receiving the notification, patients from other places should make their own accommodation arrangements while waiting for beds in the hospital.

Patients in this city must be admitted to the hospital on the same day after being notified of hospitalization. Those who cannot be admitted on time will automatically give up and re-register for bed waiting.

Family members are asked to bring valuables and cash back for safekeeping. The items provided by the hospital for patients include: hot water bottles, patient clothes and pants, a coat, a set of bedding, toilets, crutches, etc. Please keep them properly and return them when you are discharged.

Patients with infectious diseases should go to the Infectious Disease Hospital Registration Office, complete the hospitalization registration procedures (same as items 1 to 10 above), and then go home to wait for notification of hospitalization.
