Some urticaria is a type III allergic reaction. Type ⅲ allergic reaction is the reaction of antigen-antibody complex, which produces allergic toxin in the process of activating complement, stimulates mast cells to release histamine and leads to urticaria.
In addition to the above allergic reactions, some stimulating factors, such as some drugs, food, physical factors, chemicals, enzymes and tissue damage, can also directly act on mast cells to release histamine, causing urticaria.
In addition, some urticaria is also related to congenital genetic quality (ectopic).

There are common urticaria, vascular edema, artificial urticaria, solar urticaria, cold urticaria, cholinergic urticaria and other types.

1. Symptoms and diagnostic points

1) Common urticaria: ① Skin lesions often appear suddenly, which are localized red or reddish air masses with different sizes, clear boundaries and different shapes, and can be round, quasi-round or irregular. Skin lesions can increase with scratching, and can also be combined into non-plastic, grinding or annular. ② Consciously severe itching and burning sensation. ③ Most skin lesions disappear naturally from half an hour to several hours, leaving no trace after disappearance, sometimes one after another, and can recur many times in 1 day. ④ The site is uncertain, which can spread all over the body or be confined to a certain site, sometimes involving mucosa. If it occurs in the gastrointestinal tract, symptoms such as nausea, vomiting, abdominal pain and diarrhea may occur. When the laryngeal mucosa is invaded, it will cause chest tightness, asthma, dyspnea, and even suffocation and life-threatening. ⑤ Skin scratches can be positive. ⑥ According to the course of disease, it can be divided into acute and chronic. A few days to a few weeks later, acute urticaria subsided, and the reason is easy to trace. After removing the cause, it quickly subsided, and chronic urticaria recurred, often unhealed for many years, and the cause was not easy to trace. ⑦ Histopathology: Simple localized skin edema, with a small amount of lymphocytes infiltrating around the nipple, but the infiltration can also be dense and mixed with eosinophils.
2) Pseudourticaria: This disease is also called dermatoglyphic disease. ① The patient's skin is very sensitive to external mechanical stimulation, and there is usually no wheal on the skin. If you scratch your skin with nails or blunt objects, there will be strip-shaped raised scratches on the stimulation site. ② It can occur at any age without obvious reasons. Infection focus, diabetes, thyroid dysfunction, menopause and so on are often related to the onset. Trauma, clothes, friction, binding, scratches, etc. It can cause wheal and itching. ③ The course of disease is uncertain, which can last for several months or exist for a long time. (4) Some patients have subcutaneous edema after a few hours in the parts that are constantly under pressure, such as the soles of feet when walking for a long time and the hips of sedentary hard chairs, which is called stress urticaria. In addition, in rare cases, the wind mass reaction reappears in the original position half an hour to five or six hours after the dermatoglyphics subside, and can last for 48 hours, which is called delayed dermatoglyphics.
3)amgio- edema: also known as am gio neuroedema. Quink's edema or huge urticaria. ① Skin lesions are acute localized swelling with unclear boundary, normal or reddish skin color and mild itching. ② It often occurs in loose tissues such as eyelid, lip, earlobe, vulva (foreskin is the most common part) or oral cavity, tongue and throat mucosa. ③ The disease often occurs at night, only to be discovered after waking up. ④ Urticaria is often accompanied, and the damage is often single, which lasts for 2 ~ 3 days before it subsides, and often occurs repeatedly in the same part. ⑤ Some of them are hereditary, called familial angioedema, and it is a rare autosomal dominant genetic disease, which occurs in childhood. In patients' life, angioedema occurs repeatedly in the skin and mucous membrane, often confined to one or several parts, usually without wheal and itching. Often accompanied by gastrointestinal hydrops causing nausea, vomiting and severe colic, or China's throat edema causing suffocation and death.
4) Solar urticaria: part of the disease is autoimmune, that is, under the action of ultraviolet rays with a certain wavelength (below 3700A), antigenic metabolites are released to produce antibodies, and antigen-antibody reactions occur, and the other part may be caused by histamine released by phototoxicity. (1) It is common in a few minutes after sunlight exposure. Itching and erythema begin to appear at the exposed parts, and wind masses quickly form. ② It usually fades after a few minutes to 1 hour, but it can last for a long time, especially when it is caused by visible light. Some patients will develop tolerance after repeated exposure. ③ Light test is an important diagnostic method. After a short period of exposure, wheal and erythema will appear at the irradiated site.
5) Cold urticaria: There are two kinds of this disease: acquired and familial.
Familial cold urticaria is autosomal dominant inheritance, and the acquired may be autoimmune allergic reaction. Histamine release plays a certain role in the pathogenesis, which can be seen in cryoglobulinemia and paroxysmal cold-blooded egg leukemia. The characteristics of this disease are as follows: ① After contact with cold water or other cold substances, the temperature in the cold zone rises, and itchy edema and wind masses appear, which can disappear in about half an hour to about 1 hour. ② Most of them are exposed skin and face, such as face and hands. In severe cases, other parts may also be affected. Mouth, tongue, pharynx and other mucous membranes will also have edema and even abdominal pain when they encounter cold food or cold drink. ③ More common in young women. ④ It is often accompanied by headache, and in severe cases, symptoms such as hypotension and syncope may occur. Swimming in cold water or taking a cold bath may lead to shock or even death. ⑤ Put ice cubes on the skin of the patient's forearm, which can cause a typical wind mass in this part after two minutes.
Familial cold urticaria refers to the onset shortly after birth and can last for life. Often accompanied by chills, fever, headache and joint pain. Leukocytosis is a prominent feature of this disease.
6) Cholinergic urticaria: The occurrence of this disease may be that after exercise is heated or emotional, it stimulates the thermal regulation center and parasympathetic nervous system to release acetylcholine, which acts on the skin to degranulate mast cells and release histamine. ① Skin lesions showed small air masses with a diameter of 1 ~ 3 mm, surrounded by erythema, scattered and accompanied by itching. ② It often happens after heat, emotional excitement, exercise and spicy diet. Sweating and satellite-like small wind masses on erythema during the attack can be diagnosed.

The principle of treatment is to find the cause. After the corresponding treatment, we should inquire about the medical history in detail, find out and eliminate the causes, and pay special attention to gastrointestinal diseases, intestinal parasites, endocrine disorders (such as abnormal menstruation and pregnancy) and chronic diseases. Avoid eating foods and drugs that are allergic to you, avoid alcohol, strong tea, coffee, spicy and other irritating foods, and keep your stool unobstructed.
