3. Local infiltration anesthesia.
4. Cut the skin according to the drawing line, peel it off to the level of the inferior orbital margin under the subcutaneous or orbicularis oculi muscle, and stop bleeding completely.
5. Remove the flabby orbicularis oculi muscle under the incision to expose the orbital septum. If there is too much fat in the orbital septum, remove the excess fat. If the orbital septum is loose, tighten the orbital septum.
6. Overlap the flabby orbicularis oculi muscle on the lateral canthus or cut the suture 5-8mm in a wedge shape, and hang it and sew it on the lateral canthus ligament.
7. Ask the patient to look up. When the mouth is wide open, remove the excess skin and suture the skin without obvious separation of the lower eyelid. The amount of skin removed must be accurate.
8. Suture the skin with 5-0 or 6-0 suture.
