Fight rhinitis! My first surgery experience

I have suffered from rhinitis for many years, and I can’t remember exactly when it started.

Before the age of 16, I only suffered from seasonal/allergic rhinitis, nasal congestion, and runny nose. It is easy to sneeze when encountering allergens (temperature changes, dust). I don't pay much attention to it at ordinary times. I sometimes open my mouth to breathe when sleeping, and I just get over it.

I developed purulent nasal discharge after I turned 17. I especially remember the snowstorm in 2008, when the weather was extremely cold. At that time, the purulent discharge started to become serious, and because I was taking the college entrance examination that year, I didn’t pay much attention to it.

When I arrived at university, the campus environment was relatively good, with mountains and rivers, and I also participated in physical exercises. My rhinitis condition improved slightly during my college years, and my purulent discharge became less frequent. However, seasonal/allergic rhinitis still exists. When sleeping in the dormitory in winter, I have to cover my nose with a quilt for a long time before I can breathe.

After graduation and working, the condition of rhinitis began to change, gradually transforming from seasonal/allergic rhinitis to chronic rhinitis. In addition to frequent purulent discharge, there is also a smelly nose. Each lasts from a week or two to a month. Regular exercise and running can alleviate symptoms, but as work becomes increasingly busy, it is difficult to exercise every day. Factors such as working overtime and staying up late, air pollution, spicy food, bacterial and viral infections can also cause the disease to relapse.

From March to April 2014, the purulent discharge continued, and there was frequent bleeding from one nostril. Along with dizziness and other symptoms, I decided to go to the hospital.

On April 18, I went to the First Affiliated Hospital of Sun Yat-sen University (Huangpu Hospital) and was given a general specialist number. The doctor was quite responsible and did a rhinoscopy in person. The diagnosis at that time was "chronic Sinusitis, nasal polyps, deviated nasal septum”, surgical treatment is recommended. I was prescribed medicine for a week and asked to follow up for a follow-up visit in a week. After asking about the surgery fee, the doctor said it was almost 20,000 yuan and did not ask about medical insurance reimbursement.

At that time, I thought it would be better to know the condition, but I did not expect surgery, and I felt it was unnecessary. After taking the medicine for a week, the symptoms were significantly relieved and I did not go to the hospital for follow-up consultation.

But the good times did not last long. Within half a year, due to increasing work pressure, staying up late, occasionally going out to drink with friends, careless eating, air pollution and other reasons, the purulent nasal discharge reappeared. The symptoms were particularly obvious in 2015, and there was almost no month without rhinitis symptoms. During this period, I bought a lot of Renocort spray (containing glucocorticoids) on Tmall. After taking the medicine, the symptoms were slightly relieved, but the symptoms were treated instead of the root cause, and the effect became worse and worse.

In July 2015, purulent discharge with blood appeared again. Due to poor breathing through the nose, sleep is seriously affected, memory and concentration are reduced, dizziness and headaches are aggravated, and there is also a nasal odor caused by purulent mucus. Under high-intensity work pressure, tempers tend to get irritated easily, and work life has entered an almost torturous state.

In early August, it was finally decided to undergo surgery.

I came to the outpatient department of the First Affiliated Hospital of Sun Yat-sen University (Huangpu Hospital) again and called the number of Deputy Director Li Guangqi. After a brief examination and consultation, Director Li told me that my sinusitis was currently quite serious and I needed to take medication for a week to relieve the symptoms so that I could undergo surgery.

The rhinoscopy was performed by another doctor. Since I had already had a rhinoscopy last year, the doctor casually took a few photos and printed the report.

The diagnosis result is still "chronic sinusitis, nasal polyps, and deviated nasal septum", but whether all of them require surgery depends on the CT scan.

According to Director Li's description, he has performed thousands of surgeries of this kind. Any surgery has risks, but he is quite confident about the surgery, which dispelled my doubts about the surgery.

After taking the medicine for a week, I went to the hospital for a review. There were many people queuing up, and I didn’t take a fancy to it until close to noon.

After a brief examination, Director Li said that the situation had improved slightly and he was ready to go through the hospitalization procedures.

I went through the hospitalization procedures in the afternoon and paid a deposit of 3,000 yuan. While waiting in the otolaryngology inpatient department, the nurse and resident doctor asked about the medical history and condition. During this period, I learned that my attending doctor was Dr. Pei Genwang, and that Director Li and Dr. Pei performed the surgery together.

Due to the large number of hospitalized patients and limited beds, 24 beds in the corridor were temporarily allocated.

In the afternoon, after CT, chest X-ray and electrocardiogram examination. After returning to the hospital bed, I took 3 bottles of slings, one for stomach protection, one for reducing swelling, and one for anti-inflammation. Then I took one "eucalyptus, lemon and pinion enteric-coated soft capsule" (3 times a day, 1 capsule each time).

Since my family lives relatively close to each other, I haven’t been assigned a bed in the ward yet, so I went home to rest that night. Urine needs to be collected for urinalysis and blood tests the next day. No food or drink will be allowed after 22:00 in the evening.

I came to the inpatient department at 6:10 in the morning and changed into a hospital gown. I had a few injections yesterday, and my nose still has a little purulent discharge, but it's better than before.

After placing the urine in the designated location, the nurse came over and drew about 30ml of blood.

At 8:00, the nurse came over and handed out two pills, one "Latin and Loratadine dispersible tablets" (once a day, 1 tablet each time), and one "Eucalyptus, Lemon and Pinene Enteric-coated Soft Tablets" Capsules" (1 capsule 3 times a day), take immediately.

The doctor makes his rounds at 9:00. Since the CT report has not yet come out, the doctor cannot make a final diagnosis and has to wait. If there is no major problem, everything will proceed as usual.

Around 9:30, Dr. Pei called me and conducted a consultation on the medical history and condition, followed by a rhinoscopy. The results after the examination were basically the same as the previous examination. There was no sinusitis or nasal polyps on the right side of the nose.

Around 10:00, the nurse brought 4 bottles of intravenous drips, which were used just like yesterday to protect the stomach, reduce swelling, and reduce inflammation. It took about 2 hours to finish it all.

The hospital has a canteen, but I still feel like I can cook it at home. During my hospitalization, my family brought me three meals a day, and I had to go back and forth in the 35℃ high temperature every day. I would like to express my gratitude for my hard work. Got it!

After lunch, I was called to the nurse station for "surgery education." The whiteboard in the picture was written for the entire otolaryngology department.

After the surgical education, the bed was changed to bed 17, which is a two-person room with a separate bathroom. During my hospitalization, I was the only patient in this ward most of the time.

Around 15:00, Dr. Pei came over to confirm the surgery. Knowing the risks of surgery, there are cases of sinusitis surgery leading to blindness and affecting the brain. But this has never happened since Dr. Pei started working. It is usually caused by the doctor's careless operation and is an individual case. Surgery is expected to begin around 12:00. After the surgery, the nose will be stuffed with cotton and tampons, which will theoretically be removed after 24 hours. It took about 34 hours from being sent to the operating room to coming out, and the actual operation time was 11.5 hours. It may take 3 to 6 months for nose function to be fully restored after surgery, but it can usually be basically normal within 2 months. The CT report has not yet come out, and the nasal septum is slightly deviated to the right. It will be necessary to have a CT scan to determine whether nasal septum correction surgery is needed. But it is clear that sinus dilation and nasal polyp removal surgery are necessary for sinusitis.

Since the lights in the corridor outside the ward are turned on at night, and I am not used to sleeping in a hospital bed, the quality of my sleep at night is average.

At 6:30 in the morning, I was woken up by the nurse and my temperature and blood pressure were taken. Everything was normal.

Get up at around 7 o'clock, wash up, and then stay in bed. During this period, the doctor came over for inspection and informed me that the CT report had revealed a high nasal septum deviation. I took my temperature again around 10 o'clock and it was normal.

Until 11:15, the nurse informed me to go to the toilet, give me a pre-operative injection, and then I was wheeled by a nurse to the surgery area on the 9th floor, with my family accompanying me during this period.

I waited in the operating area for half an hour from 11:30 to 12:10. During this period, a doctor came to help administer an intravenous drip that should be invert sugar, and inserted a reusable infusion needle into my left hand.

12:00 I was pushed into operating room 6 by a staff member, and I was greeted by the anesthesiologist and staff. I lay down on the operating table, and the staff and anesthesiologist began to prepare for the operation.

During the period, the anesthesiologist asked my name, age, weight, etc. He told me that during the next anesthesia, I would fall asleep within 15 seconds, and a life-sustaining oxygen tube would be inserted into my mouth. When I wake up, the operation will be completed. The oxygen tube may be a little choked by then, so just remember to take a deep breath. And asked me to sign the anesthesia confirmation form.

Immediately afterwards, they continued with preoperative preparations. The relaxed atmosphere in the entire operating room can be felt from their conversation. Then Dr. Pei and Director Li entered the operating room. Dr. Li said that because the nasal septum was severely deviated to the right, nasal septum correction surgery was needed, "Let him feel better!".

I felt very relaxed, not afraid of the surgery, and was curious about the postoperative situation.

The anesthesiologist put an oxygen mask on me and asked me to take a deep breath. After a while, the anesthesia took effect and I felt my eyes closing involuntarily. At the time, I felt that it all happened very subtly and suddenly. For a few seconds, I thought about opening my eyes to try, but my desire for recovery gave up the idea. After my eyes were completely closed, I fell unconscious and completely entered a state similar to deep sleep.

I don’t know how long it took, but I woke up suddenly, feeling like I was woken up by the anesthesiologist. Immediately I felt a little choked by the tube inserted in my throat. Upon seeing this, the doctor asked me to take a deep breath and then pulled out the tube. At this time, my two nostrils were filled with cotton and I could only breathe through my mouth. However, I did not feel any pain. I just felt that I had been lying down for a while and my limbs were a little stiff.

Next I was lifted onto a single rack and sent to the resuscitation room for observation. At this time I saw that the time was about 15:15. From time to time, blood will flow into the throat, and it is necessary to ask someone to help vomit it up.

After observing me in the recovery room for a few minutes, Dr. Pei pushed me out of the operating area and prepared to return to the ward. My family waiting outside the surgery area was relieved to see that I was fine.

Returning to the ward, at about 15:45, we started to perform 6 hours of ECG monitoring, oxygen inhalation, anti-swollen, anti-inflammatory, gastric protection, invert sugar, and glucose bottles. You can only rely on pillows and lie flat on your back. During this period, the right nostril was bleeding heavily, and the nurse called the doctor to stuff another piece of hemostatic cotton into the right nose.

While listening to TV and chatting with my family, time flies by. 9:45 You can get up to drink water and eat. Since you can only breathe through your mouth, your mouth is very dry and you need to replenish water in time.

That night, I slept half-sitting, because there was still blood oozing from my nose, and I had to let the blood flow out. My nose was stuffed with tampons and cotton, and I could only breathe through my mouth. It was difficult to sleep well. Blood came out twice in the middle of the night. I wiped it clean with a tissue and continued to sleep.

It is still necessary to take medicine regularly, take your temperature, and put in a sling bottle. The doctor said that there was a lot of bleeding yesterday and the tampon could not be removed yet.

Actually, today is the most uncomfortable day during my hospitalization. Because I didn't sleep well, my head was a little dizzy, and my glasses were a little blurry in the distance. Because the nose is stuffed with stuff, which compresses the tear ducts, tears will flow out from time to time, especially the cotton stuffed in the right nostril the day before. Blood seeps out from time to time, and the eyes and face will feel a little swollen. The swelling makes it difficult to fall asleep at night. , and when I went to bed that night, there was more blood oozing out in the middle of the night.

Continue to take medicine on a regular basis, take your temperature, and put in a sling bottle. Dr. Pei made his rounds and said that I could remove the tampon today. He also said that I produced a lot of blood during the operation, about 150ml. Normally, this kind of surgery would not produce that much blood.

I arrived at the nasal endoscopy room at around 10 o'clock, and Dr. Pei performed the procedure himself. In my memory, I pulled out about 5 to 6 tampons of about 10cm. When I pulled them out, I felt so sore that I burst into tears. If you don’t pull it out, you won’t know that so many things can be stuffed into your nose!

"The recovery was better than imagined," Dr. Pei said. Since the nose showed signs of bleeding again after removing the tampon, two more pieces of hemostatic cotton were inserted. But this is much better than the previous two days, and my nose can be relieved a little. But after a while, my nose was clogged with secretions again. Just like this, I watched movies and TV for a day between ventilation and non-ventilation.

When I woke up in the morning, my nose was breathable again, and I felt like I had a good night's sleep. But not long after getting up, my nose was blocked by secretions again.

Continue to take medicine on a regular basis, take your temperature, and put in a sling bottle. Watched TV and movies all day.

I went to bed at 23:00 last night and fell asleep around 0:00. I woke up at 5:30 in the morning, but I felt that my sleep was deep. My head is still a little dizzy but much better than the day before, and my eyes are still a little blurry. However, the nose has been ventilated and you can breathe completely through the nose.

I continued to inject the bottle and take medicine. During the rounds, the doctor told me to clean my nasal cavity today. If there are no problems, I can be discharged from the hospital tomorrow.

At around 5 p.m., Dr. Pei asked me to go to the nasal endoscopy room to clean my nose. This time, I removed the cotton stuffed the day before and sucked out the blood scab and other foreign matter in the nose. Dr. Pei said There are also some biodegradable absorbent cottons that are placed in the nose and do not need to be taken out. There may still be blood and secretions afterward, which is normal.

After this cleaning, my nose was completely clear. It’s smoother than I’ve ever tried in years!

I slept well that night.

The day of discharge finally came. I went to the outpatient clinic in the morning. Director Li checked it and took out some blood scabs and said that after today’s injection, he could be discharged from the hospital. He would bring medicine with him when he was discharged (still in the hospital). What kind of medicines are you taking). There will still be discharge from the nose within a month, which is normal. Since there is still bleeding, I will take the medicine first in the first week without nasal spray or nasal rinse. You can engage in normal physical activity, but do not touch your nose.

After lunch, the discharge procedures were completed and relevant report materials were copied.

After resting at home for a day, I returned to work on the 27th.

On the 28th, my left nose bled for more than an hour for unknown reasons. After being blocked for 2 hours, I cleaned it up and returned to normal.

A week later, on September 1st, I had a reexamination. I had a rhinoscopy and my nose was cleaned. Everything was normal. The doctor said that you can start nasal washing and use Renacote nasal spray twice a day, 2 sprays each time. He also prescribed Biyuanshu oral liquid 3 times a day, 1 bottle each time and Desloratadine citrate tablets (Beixue). ) 1 tablet per day.

Now I insist on washing my nose every day. I bought the nasal washing machine and nasal washing salt on Tmall Ouchang. After washing my nose, I feel very smooth and comfortable.

I mentioned the problems of dizziness and blurred vision before, but they basically returned to normal after a few days of rest after being discharged from the hospital.

Two weeks later, on September 15, I went to the hospital for another review. This time I also had a rhinoscopy and cleaning. Lei Ning, mometasone furoate nasal spray and Biyuan Tongqiao granules were prescribed. After using up the medicine this time, there are no other nose-related problems such as colds, so there is no need for further check-ups.

Now my nose remains clear every day, and I sleep well at night. I wash my nose 1 to 2 times a day. I also developed the habit of going to bed early and getting up early, paying attention to my diet, and exercising regularly! Health is really important! Without health, no matter how much money you make, it’s useless! Without health, it affects not only yourself, but also those around you.

If there is anything wrong with your body, you must go to the hospital. Don't be too troublesome. Health is more important than anything else!
