Uterine fibroids, known as "the first gynecological tumor", are an important reason that affects the health of modern women. It is the most common benign tumor of female reproductive system. Most patients are not found because of their small size and asymptomatic, and they are usually found during physical examination. The size and nature of hysteromyoma are influenced by estrogen, and its harm to women is also related to the level of estrogen.

Women with uterine fibroids may have increased menstrual flow and irregular vaginal bleeding, mostly in submucosal and intramural fibroids. When the myoma increases, the abdomen can feel the lump, the abdomen has compression symptoms, and the vaginal discharge will increase, because the compression of the myoma is twisted and sometimes there will be pain.

There are many treatments for uterine fibroids, depending on the age, symptoms, size and nature of the fibroids. So what are the treatments for fibroids? (1) Follow-up observation: Myoma less than 8 weeks pregnant without obvious symptoms, or uterus at 12 weeks pregnant with normal menstruation without compression symptoms can be observed temporarily. Adhere to the review every 3 months. Generally, after menopause, because of the decrease of estrogen level, fibroids can gradually shrink. During the follow-up period, if the myoma is enlarged or the symptoms are obvious, the possibility of malignant transformation of the myoma should be considered. Surgical treatment should be carried out in time.

(2) Drug therapy: For patients with menorrhagia and uterus enlargement for about 8 weeks, after endometrial cancer is excluded by diagnostic curettage, (1) androgen therapy can be used. Androgen can resist estrogen, promote endometrial atrophy, contract muscular layer and vascular smooth muscle, and reduce bleeding. Methyltestosterone and testosterone propionate are commonly used, but the dosage and method should be under the guidance of a doctor to avoid improper medication, causing endocrine disorders and masculinity. (2) Gonadotropin-releasing hormone agonist (Gnrha) can shrink the myoma by 40% ~ 70% with little side effect. If it is a large myoma and the operation is difficult, this drug can shrink the myoma before operation, but its disadvantage is that it is more expensive. (3) Mifepristone can shrink fibroids and needs to be taken every day for three months. However, this drug is prone to hormone dependence, and the fibroids will grow up after stopping the drug, which is not a good radical cure.

(3) Interventional treatment of hysteromyoma: Interventional treatment of hysteromyoma is to insert a very thin catheter into uterine artery by non-surgical method to block the blood supply of hysteromyoma, so that it loses nutrition and dies and shrinks on its own.

(4) Surgical treatment: those who are ineffective after long-term conservative treatment, or those with obvious symptoms, large fibroids, anemia and rapid growth should be considered for treatment.

1. myomectomy: suitable for young patients who want to have children. Whether it is subserous myoma, intermuscular myoma or even submucosal myoma, it can be removed by abdomen to preserve the uterus.

2. Hysterectomy: Total hysterectomy, subtotal hysterectomy and vaginal hysterectomy should be performed for elderly patients with hysteromyoma who have obvious symptoms and have no requirements for continued fertility. At the age of 50, a normal ovary can be preserved to maintain its endocrine function.

3. Minimally invasive treatment of uterine fibroids: a new breakthrough in treatment. The use of minimally invasive treatment has solved the big problem of women. In the traditional hysteromyoma surgery, doctors cut a 10cm or even longer incision in the patient's abdomen, which caused great damage, much bleeding and poor prognosis. Laparoscopic surgery is skilled in our hospital. It only needs to cut three small holes of several millimeters in the abdomen of the patient, and the operation can be carried out through the monitoring screen, which is basically harmless to other parts of the patient and has high operation accuracy. The application of minimally invasive technology in gynecological hysteromyoma surgery is another great progress in medicine. Traditional gynecological surgery generally requires abdominal incision and hospitalization for at least one week after operation. The application of minimally invasive technology in gynecology has brought convenience and progress to gynecological treatment. Except caesarean section, any gynecological disease can be treated with minimally invasive surgery and minimally invasive treatment. Such as: hysteromyoma, ovarian cyst, endometriosis, fallopian tube recanalization, hysterectomy and so on. These operations can be completed only by opening 3 ~ 4 small holes of several millimeters in the abdomen, with little trauma, quick recovery and almost no scars after operation. Minimally invasive treatment has become a better choice for many women to treat gynecological diseases.
