The filing certificate generally includes the following contents:
1, basic information such as clinic name, address and contact information;
2, the clinic registration certificate number and validity;
3, the specific content and scope of outpatient record, such as diagnosis and treatment subjects, clients, staffing, etc. ;
4, outpatient record audit department and audit time and other information.
The clinic shall submit the following materials for the record:
1, clinic filing information form;
2, outpatient building layout (refers to the outpatient room in accordance with the proportion of identification, indicating the functional distribution and area size);
3. Property right certificate or lease contract of the clinic;
4, the legal representative of the clinic, the main person in charge of the valid identity certificate and a copy of the relevant qualification certificate and practice certificate;
5, other health technical personnel directory, valid identity certificate and relevant qualification certificate, a copy of the practicing certificate;
6, outpatient rules and regulations;
7, outpatient equipment list;
8, attached to the pharmacy (cabinet) drug list;
9, outpatient sewage, dirt, feces treatment scheme, outpatient surrounding environment;
10, other relevant materials required by laws and regulations.
To sum up, the filing certificate is a necessary proof of the legal operation of the clinic and an important basis for trust and cooperation between the clinic and patients. At the same time, the registration certificate is also a monitoring tool of the health administrative department for the legal operation and management of the clinic, which helps to ensure the health and safety of the public.

Article 4 of the Interim Measures for the Administration of Filing in Chinese Medicine Clinics
Hold a Chinese medicine clinic, to be held at the county level Chinese medicine department for the record, can carry out practice activities.
Article 5
Chinese medicine clinics shall meet the following conditions at the same time:
(1) An individual who runs a Chinese medicine clinic shall have a Chinese medicine practice qualification certificate and have been practicing in a medical, preventive and health care institution for three years after registration, or have a Chinese medicine (specialist) practice qualification certificate; Where a legal person or other organization holds a Chinese medicine clinic, the main person in charge of the clinic shall meet the above requirements;
(two) in line with the "basic standards of Chinese medicine clinics";
(three) the name of the Chinese medicine clinic conforms to the relevant provisions of the detailed rules for the implementation of the regulations on the administration of medical institutions;
(four) in accordance with the relevant provisions of environmental protection and fire protection;
(5) Being able to bear civil liability independently.
The Detailed Rules for the Implementation of the Regulations on the Administration of Medical Institutions stipulates that no unit or individual may apply for setting up a medical institution or holding a Chinese medicine clinic.
