To register as an assistant practicing physician, it is necessary to provide a review form of the Application Form for the Registration of Physician Practice, a copy of the second-generation ID card of residents, a copy of the qualification certificate of the assistant practicing physician, a recent two-inch bareheaded frontal bust photo, and a job certificate of the medical institution to be practiced.

Approval Form for Application for Registration of Medical Practitioners (2 copies);

Copy of the second-generation ID card of residents (the front and back of the ID card are copied on the same side of a page); Copy of doctor's qualification certificate (photo and name are copied on both sides of a page);

Employment certificate of the medical institution to be practiced (indicating the discipline (specialty) and employment period);

Recent two-inch bareheaded frontal half-length photos 1 piece (except those posted on the application form); Proof of the applicant within 6 months;

The original and photocopy of the practicing certificate of the practicing assistant;

Proof that the original practice institution agrees to change to a registered medical practitioner of other units (applicable to the situation that the assistant medical practitioner is registered in the Committee, but different from the registered medical practitioner, the original work unit needs to agree to change);
