Excessive examination and treatment are the root cause of conflicts between doctors and patients. What do you think about this?

Hello everyone, I am a doctor, and I have been a patient in the hospital before, so I will talk about my views from the perspective of the general public.

The question of excessive examination and treatment does exist in hospitals. We cannot deny that we should look directly at this problem.

Let me share a case first: When I was in college, I was working part-time outside the school. One day a student came to class with a bruise on her head. I asked her what was wrong, and she said she woke up after sleeping. Bumped on the bed. She asked if she could take leave to go to the hospital for a checkup. I said it was not necessary, just take a rest. But she still went to the hospital. The doctor took a CT scan and prescribed drugs to promote blood circulation and remove blood stasis, which cost more than 500 yuan. Unfortunately, these checks are completely unnecessary.

In the past, doctors were respected, but now the relationship between doctors and patients is really tense, especially some reports of medical troubles, which have further exposed this problem.

I think the current doctor-patient relationship often has the following reasons:

First, doctors or nurses have an indifferent or unfriendly attitude toward patients. I also encountered this during my visits to the hospital. The doctor was unkind to me.

Second, excessive examination and treatment are serious. Friends around me said that it would cost at least several thousand yuan to go to the hospital for a cold, and tens of thousands of yuan to be hospitalized. The average person's monthly salary is only a few thousand yuan, so they really can't afford to get sick.

Third, doctors explain hospital-related issues to patients simply and rudely. Patients have an extremely poor grasp of medical knowledge, and medicine takes the absolute initiative. Patients hope to get more medical knowledge and medical advice from doctors, but doctors give patients too little. For example, the consultation process only lasts a few minutes. How can this be diagnosed and treated? Are you satisfied?

Fourth, patients’ expectations for medical treatment are too high. Currently, medicine can solve only a small part of the problems, but patients also hope to survive longer. There must be contradictions in this.

Fifth, the social environment is stressful. Some friends have some negative emotions. After getting sick, these negative emotions are aggravated. They have a bad attitude towards doctors, which intensifies themselves.

I think it should be solved like this:

Second, establish an evaluation system for doctors’ attitudes, and let patients rate doctors.

Third, increase investment in medical insurance to make medical treatment affordable for the public.

Of course, some people say that in order to put an end to medical trouble, those who make medical trouble must be severely punished and so on. I personally do not support this, because merely blocking the problem is not a good way to solve the problem. We should face up to the problem and solve the pain points of the problem, so as to fundamentally prevent medical trouble from happening again. I also hope that everyone can have a better understanding of doctors. After all, it is not easy for everyone.

The above is my opinion expressed from the perspective of an ordinary public. I am a medical worker who works on AIDS prevention and treatment. My patients or consultants like me because I am friendly to them and can listen to their good or bad ideas. So I also hope that all medical workers can have a better attitude towards patients. People's hearts are full of flesh and blood, and no one is a bad person from the beginning.

Excessive examination and treatment are just the symptoms of the doctor-patient relationship. I think the underlying problem is that the positioning and development of medical institutions within the existing system has reached a dead end.

Because my late wife suffered from cancer, I have been interacting with hospitals and doctors for more than three years and have a certain understanding of hospitals and doctors. I will now express my feelings from an objective standpoint. If there is anything wrong, please correct me!

Let’s talk about the patient first. A minor illness is excruciatingly expensive, but a serious illness is excruciatingly expensive! When ordinary people get sick, they go to small hospitals for fear of misdiagnosis, and go to big hospitals for fear of queuing! It’s either a blood test or a CT scan. The final prescription for diagnosis is to prescribe some medicine if you don’t need medicine, two injections if you don’t need injections, some fluids if you don’t need fluids, and “recommend” that you be hospitalized if you don’t need hospitalization! In short, the feeling is that since you are here, you should pay more! If you have medical insurance, pay more. I am also "responsible" for your health. This is what a patient feels!

In fact, hospitals are also in a difficult situation! Hospitals are not only responsible for their own patients, they are also responsible for their managers and the public! Every hospital has its own safety indicators. If someone dies due to misdiagnosis, all medical staff in the hospital will be implicated. This is also the driving force why hospitals hope to use the best medicine to cure patients as soon as possible. In fact, this point can still be understood by hospitals, but hospitals also need to develop, be promoted, and be evaluated for excellence. Only by gaining reputation through promotion can we introduce higher-quality talents (doctors), and through development can we better improve medical conditions and better serve patients. So the question is – where does the money come from? In the past, medicine was used to support medicine, but now medical examination is used to support medicine! However, the use of medicine to support medical treatment has not been completely discontinued. So it feels like it is not only more expensive than before, but also more expensive and troublesome than before.

As for doctors, I think it’s not easy for them. Every day, I have to face many outpatients and patients in inpatient beds; not only do I have to sit down and write copywriting, but I also have to go to the operating table to perform operations; not only do I have to treat illnesses and save lives, but I also have to summarize cases and conduct scientific research and innovation. It's not easy.

The reason why doctors have sharp conflicts with patients may appear to be that the doctors are not patient enough, but in fact it is a sign of the doctors being overstressed! There are several pressures here: 1. Time pressure. Doctors’ time is not only competing with death, but also competing with tasks, time competing with evaluations, time competing with promotions, and time competing with papers; 2. As mentioned above pressure of responsibility. Faced with many patients a day, if a medical accident occurs, you, your colleagues, and the hospital will all be implicated. This responsibility is not small. 3. Pressure for career advancement. If you want to be promoted, you must complete various indicators set by the hospital and the Health and Family Planning Commission. Among them are the hospital’s revenue-generating indicators! This is where conflicts between patients and doctors are most likely to arise!

I once saw a medical incident. The situation was probably that a patient had a cough and fever. The doctor asked the patient to take a B-ultrasound. The doctor found a small shadow, so he asked the patient to take a color ultrasound. This time It showed that no problem was found, so the doctor prescribed a prescription based on inflammation, but the patient was not happy! The doctor said to continue the investigation, but the patient was even more unhappy! So it started.

The reflection this incident gave me is that both the patient and the hospital (doctor) are entangled in health and money. Although the hospital (doctor) recommends patients to review the examination in a responsible manner and has good intentions to eliminate hidden dangers, it cannot rule out the fact that the examination is used to generate income, so in the final analysis it is still a question of money.

The development of hospitals, the remuneration of medical staff, the upgrading of medical equipment, the maintenance of medical venues, and medical scientific research are all major sources of money. If the funding problem required for hospital development can be solved institutionally, hospitals and doctors can focus on treating diseases and saving lives and doing academic research, with more low-priced drugs and fewer examinations. I think this is the ultimate way to fundamentally resolve the contradiction between doctors and patients!

By the way, pharmaceutical companies and equipment manufacturers are also inseparable from the contradiction between doctors and patients. Let’s talk about it later when we have time!

I went to the largest hospital in the province to treat oral ulcers because I lived near the hospital. As a result, the experts induced me to think about the worst cancer and let you think about it yourself. They also said that I must be hospitalized. Experts from the provincial hospital all said that we should be hospitalized. It turned out that the various expenses on the first day of hospitalization were 5,300 yuan. I am a laid-off worker in my 50s. Can I bear the cost? It was dark on the second day of hospitalization, so I thought Before the doctor at the hospital went to work for rounds, I took a taxi to an ordinary municipal hospital to check again. I took a taxi and went there. I was the first to register. The doctor said that this is the most common disease and it will be cured without taking medicine. It just takes time. After a few days, I was prescribed more than 60 yuan of medicine. I immediately took a taxi back to the provincial hospital where I was hospitalized and was discharged home. It took more than a week for me to recover. I was holding the hospitalization invoice, and I was so angry that I wanted to kill someone.

Treatment of humeral fracture may cost 60,000 yuan or 1,000 yuan!

From the fall on September 20th to the present day, when I write this article, my right hand is still hanging, and my right arm is wearing a heavy external fixation brace.

I would like to use my own personal experience to illustrate this issue, wouldn’t it be more convincing?

Not slanderous, just faithful record.

The traditional Chinese medicine hospital refused to provide treatment

On the morning of September 20, after traveling around for many years, I sadly ended up in the toilet of a hotel out of town. . . . . .

A somersault on his back inexplicably broke the humerus of his right arm.

After examination at the local hospital, the doctor asked me to choose conservative treatment or surgical treatment.

After consideration, we decided to simply fix it and then return to Beijing for treatment. The first choice was conservative treatment at a famous traditional Chinese medicine hospital in Beijing.

After running all the way and insisting on returning to Beijing, he was hit in the head at the Traditional Chinese Medicine Hospital.

The emergency doctor said that my fracture was very serious and they were unable to treat it. I had to go to the best orthopedic hospital in Beijing as soon as possible. . . . . .

Send him away directly!

Later I understood why they did this, but at that time, I was suddenly frightened by this doctor.

Conservative treatment is no longer the first option.

The 60,000 yuan surgery fee made me a little confused

Not daring to delay, we immediately rushed to the most famous orthopedic hospital in Beijing and went to the emergency room.

The doctor looked at the X-rays we brought and said easily: It’s just a minor surgery, it’s not serious.

My little heart, which was frightened by the Chinese medicine doctor, felt a little more at ease.

Immediately, I took a sheet with the adherence process and started to do the pre-surgery physical examination.

The consultation lasted about five minutes. During this period, other patients came in and out. The doctor was very busy and did not say a few words.

After the examination, it was agreed that he would be hospitalized for surgery three days later and wait for the nucleic acid test results.

We asked about the cost of the surgery and were told that the deposit was 10,000 yuan.

We estimate that the cost of treatment should be within this range and not expensive.

I was successfully admitted to the hospital three days later. During this period, a doctor came to ask the family members to sign, and then a doctor came to ask the family members to sign, as if the family members were setting up tents downstairs.

There was a thunderbolt on a sunny night, and the nurse suddenly informed me: At nine o'clock tomorrow morning, I asked my family to bring 60,000 yuan for the surgery.

I'm a little confused.

Decision and Uneasiness

In order not to disturb others’ rest, I took the phone and ran to the corridor.

First, notify your family to prepare the surgery fee. This is simple. At worst, you may have to pay your credit card first.

Then I asked what the 60,000 yuan was.

The nurse was well-informed and said with a smile that an imported steel nail costs 6,000 to 7,000 yuan, and the surgery would cost 50,000 to 60,000 yuan.

The hospital performs at least 1,000 surgeries every week.

I asked the doctor on duty if there were any domestic steel nails, and the answer was that they were all imported.

I asked an insurance company friend to check the reimbursement ratio, which was 65% for domestic products and very low for imported products.

After all, I’m afraid it will cost tens of thousands of yuan out of pocket.

Then there are all kinds of harassing friends, taking X-rays and asking them to find someone to ask if they can be treated conservatively.

After a night of no results, I kept checking the Internet for information. Thank you to the Internet.

The next morning, the dean personally inspected the ward. When he came to me, he had nothing to say: There is nothing to say about this (original words)

Friends also responded with news: It is recommended to be conservative treat.

After hesitating again and again, I made up my mind to be discharged from the hospital!

How much is the hard work of the three doctors worth?

I was prepared for a rainy day and called up another famous orthopedic hospital in Beijing the next day, but it didn’t work.

Before discharge, the doctor said responsibly: I have to adjust your condition before I can discharge you. I cannot discharge you without giving you treatment.

I tentatively asked if they could just give me conservative treatment, and the doctor agreed.

So in the spirit of not bothering the two masters, I treated him here!

I also changed hospitals because I was tired of it.

The doctor suggested that I don’t need a plaster to make a brace, but it would be at my own expense, more than 1,000 yuan, which is acceptable.

Then three doctors came to make the brace, which took about three hours and delayed lunch.

During the installation, they set my bones and took about three minutes. After the installation, I took X-rays and the position was very good.

It was agreed that a re-examination would be carried out in one week and the patient would be discharged home. The brace fee would be 890 yuan.

Now I understand why Chinese medicine practitioners don’t take this job.

The steel nail materials cost tens of thousands, and the braces cost 890 yuan for several doctors to work for a long time. The craftsmanship is really worthless.

Is there excessive medical treatment?

In a few days, the fracture will be four weeks old, and I feel okay.

At least I can use my right hand when writing on my mobile phone.

No longer like a while ago, you can only operate it with your left hand.

From a personal perspective and objectively speaking, surgical treatment and conservative treatment have their own pros and cons.

Since the humerus is long and difficult to fix, I did feel this. In the initial stage, I felt a little bit of a stretch when I sneezed.

It is a fact that surgical treatment can shorten the time for patients who cannot take care of themselves, has good fixation effect, and has high certainty.

However, the damage to the body is also greater, and it also involves the issue of whether to remove steel nails in the future.

Conservative treatment takes a long time to recover, there are many unexpected factors, and poor recovery may cause functional damage.

The advantage is that it saves money and does little harm to the body.

Frankly speaking, it must be said that he has been over-medicated. I am afraid it is quite controversial, especially the doctors who definitely do not support this view.

Things in the world are inherently complex and not black and white.

I don’t think it’s over-medicalization. This kind of thing has different personal perspectives, so people have different opinions!

I still feel a little uncomfortable

From my personal experience, at least the following points are indeed worthy of discussion.

The first is the patient’s right to know. From my personal experience

I see that this aspect is not satisfactory.

The traditional Chinese medicine hospital adopted a simple evasion method and scared me away with scaremongering.

The orthopedic hospital did not let me choose between conservative treatment or surgical treatment at all, and directly asked me to follow the surgical procedure.

As for the treatment plan and cost, no one told me.

Don’t tell me that it’s impossible for the hospital to do this. I can take responsibility for what I say. It’s all the truth.

The second issue is material selection. According to the information I later inquired about, many other hospitals in Beijing can choose domestic steel nails or imported steel nails, which is directly related to the proportion of medical insurance reimbursement!

Without a choice, the patient is likely to have to pay tens of thousands to tens of thousands of yuan more out of pocket.

The third is that the medical treatment process is unreasonable. Fortunately, I have a broken arm and can still walk through the examination process like walking a monkey. If my leg is injured, it will be a torment.

When you are hospitalized and cannot go out, you don’t know when you will have to look for your family members. It seems like your family members are always outside waiting for a call 24 hours a day.

These situations may ultimately make patients and their families very unhappy and affect their emotions.

As a result, the doctor felt that he was not being thanked for his hard work, and the patient was also complaining that he could not find any gratitude.

Both sides feel wronged.

My physical fitness is pretty good, and I rarely catch a cold.

I haven’t entered the hospital gate for many years, so I only heard about it from hearsay.

I experienced it personally this time and was quite speechless.

Doctors are actually good doctors. Doctors in regular hospitals can still be trusted, not those in Putian.

Hospitals naturally have their own difficulties, and doctors also have the right to pursue a better life, so they cannot be completely harsh.

I hope that I will feel better and better when I see a doctor in the future!

My mother had frequent diarrhea recently. She went to the hospital to see a doctor. The doctor prescribed a test list, including electrocardiogram, color ultrasound, gastroscopy, and CT. After the examination, she spent nearly 2,000. The doctor said that the CT test sheet contained kidney stones. , there are no other problems, why not do a colonoscopy? Doctors are professional in treating diseases. We don’t understand it, but it is reasonable if it exists, so the doctor is treating the disease by ruling out the law. If this is a sign of a serious illness, just one examination can do it. You can see it very clearly, no problem, everyone can rest assured. But I think there is nothing wrong with the examinations, and neither are the doctors. Why are the fees for a series of examinations so high? Well, your equipment has a cost, but when it comes to cost recovery, do public hospitals only have patients pay for consumables and provide the rest for free? ? Healthcare reform is a top priority, not who should be held responsible when problems arise. Because in too many families, if they get sick, the treatment will result in the death of their families. I have seen a news before. A 50-year-old man in a rural area fell ill. When he learned that the medical expenses were hundreds of thousands, he committed suicide by drowning himself in a river.

Is excessive examination and treatment the root of the conflict between doctors and patients?

It can be said to be one of the contradictions between doctors and patients. Excessive medical treatment is definitely not appropriate. This is an unethical behavior. If it goes too far, it may constitute a crime of fraud. However, in reality, doctors can only A few among the few violate medical ethics and put personal interests first, causing patients to bear unnecessary expenses and increasing the burden. They should be condemned by conscience and make deep introspection. They should also wake up and make thorough corrections.

Doctors are a special profession, and it is their bounden duty to save lives and heal the wounded. Both doctors and patients should give in to each other and jointly maintain order in the medical field to achieve a win-win situation.

The relationship between doctors and patients is tense, and excessive medical treatment is definitely a key factor. Take my own ring removal surgery!

I went to the hospital to have my ring removed in the spring of 2015. The slow process made me unable to eat or sleep. I went to the hospital 6 times in one day, but I didn't even take it off. Every time I went there, I was asked to take blood tests and drink 2 bottles of mineral water for a B-ultrasound. When I felt like I had urine and couldn't hold it in, I was called for a B-ultrasound. When it came time to actually remove the ring, no matter whether it was in the outpatient department or the inpatient department, due to the limited professional skills of the doctors, they did not even put in the medicine to dilate the cervix, so it was impossible to remove the ring at all. This made me frustrated and frustrated. He said, I was so scared that I couldn’t eat or sleep, and even threatened me that if I couldn’t remove the ring, I would have to undergo hysteroscopic surgery. Oh my God, I wasn’t skilled enough and wanted to harm the patient. I was so scared that I ran away and didn’t dare to remove the ring. Six various inspections cost nearly 2,000 yuan, but nothing worked.

Later, a friend who was helping me find a job recommended a town gynecologist. The friend said that her mother was also in the hospital where I had my ring removed, but she couldn’t get it off. She gave me all kinds of threats. Later, someone also recommended me to the town hospital. Successfully removed the ring.

After receiving her advice, I didn’t even eat, so I quickly asked my female classmate to accompany me on the bus to the town hospital. When I arrived at the town hospital and found the doctor I was looking for, she sterilized the medical bag. , the hospital did not have a disposable medical kit, so the doctor put the uterine dilation medicine on me and asked me to go for a walk with my classmates in the afternoon.

My classmates and I had a good meal and walked for a while before returning to the town hospital. The doctor asked me to go to the delivery bed and used various medical instruments for removing the IUD. After sweating and some hard work, the ring was finally removed. I am grateful to the doctors from the town hospital who are highly skilled in medical technology. They relieved me of my biggest worries. They did not conduct any examinations and saved a lot of money. The service attitude was good and the patient felt relaxed. .

A good doctor is a relative of the patient. Excellent medical skills can ease patients' worries.

Two weeks ago, my mother had kidney stones. She went to the hospital. After registration, the doctor asked a few questions, followed by color ultrasound, CT, blood tests, and urine tests. It hurts. I queued up to register at 7 o'clock in the morning and didn't ask for gravel until noon. I couldn't get a seat for gravel. I said I would make an appointment for the next time, but the pain was so bad that I couldn't even straighten up even after taking the painkiller injection. Isn't this a scam? I won’t see what’s going on with the color ultrasound. Is the instillation wrong? The results of the blood test and urine test are slow to come out. If the results don’t come out, it won’t affect the treatment. I have to wait for the results to come out before giving treatment. My mother has a good temper. Degu was so angry that he didn't care if some people couldn't stand it and died of pain. Later, he took the prescribed medicine and went to a clinic in the community to find an acquaintance and inject fluids to relieve the pain. I have to wait at the hospital. I don't know when they will give me fluids. Really speechless! It’s also my fault that I didn’t find an acquaintance, thinking that my son also had stones last year but was very small. I asked an acquaintance to do a color ultrasound and immediately told me to go to the drugstore to buy Pai Shi granules and go home and drink water. alright. This time I thought it was so simple, but I knew it was so tricky! After remission, I went to a larger hospital in the city and found an acquaintance who was more professional. The problem was solved in half a day. The more professional person said that there was no need to do a CT scan or anything like that. It was unnecessary.

Regarding this point, I clearly remember that I had a cervical spondylosis that caused dry eyes due to over-organizing notes. Since I'm busy at work and it's close to the hospital, my colleagues said you should go and have a look! I went to the hospital, and an old lady doctor with white hair saw me and asked me what was wrong. I said that my eyes always feel achy at eight or nine o'clock every morning. The old lady doctor directly said to pat my cervical spine. ! I said I was busy. If you want to be sure, just give me the medicine! In fact, it’s still right to make a movie! But the doctor smiled and gave me the medicine, and I was fine within a few days! While thanking you for your understanding, we doctors ourselves should also strengthen the training of medical skills and not always rely entirely on instruments. As people say, is it your doctor who sees the doctor or the instrument? Of course necessary examinations are necessary. Why should we waste patients’ money when examinations are not necessary? No wonder repair shops and car repair shops follow your example. It’s painful for patients to smile in front of you! Sickness and heartache! They are all clenching their teeth and pretending to smile, just to make you happy and cure the disease! ...

There is indeed a certain truth to this. Nowadays, hospitals require routine examinations whenever they see a doctor. This will inevitably lead to patients questioning and thinking that doctors are doing it to make money. This is also because The hospital collects remuneration for doctors' prescriptions. Excessive examinations make patients conflict with medical ethics, which also allows doctors to develop selfishness and affect professional ethics.

In ancient times, the famous doctor Bian Que could diagnose the disease by "looking, smelling, asking, and feeling". Don't today's doctors learn this at all? I think:

First: If you need to check to confirm the condition, you must go through the examination method. This cannot be doubted and denied, and it is enough to communicate with the patient.

It is: For ordinary minor illnesses, patients do not need to undergo comprehensive examinations of this or that complexity. A minor illness often costs several hundred yuan in check-up fees. How can ordinary people afford it? This will undoubtedly lead to conflicts between patients and doctors, resulting in excessive violence.
