Is circumcision a minor operation?
Excessive prepuce is a common male disease with a high incidence. With the improvement of people's understanding of excessive prepuce, more and more patients require surgical treatment. Circumcision is a common minor outpatient operation, but if the operation is rough, it will cause many postoperative complications. Now introduce the prevention and treatment of complications. When you read the following patiently, you will believe that circumcision is really not simple. First, postoperative bleeding is a common complication. Common bleeding sites: (1) Bleeding at the prepuce frenulum is usually caused by incomplete hemostasis during operation or ligation line falling off after penis erection at night. Because the frenulum is rich in blood supply, it is difficult to stop bleeding completely, so it is a common bleeding site, which is often manifested as blood flowing out from the self-created mouth, forming a large subcutaneous hematoma. Those who bleed out from the self-created mouth do 8-shaped suture at the incision to stop bleeding when they re-operate. For the large hematoma, immediately remove the suture to remove the blood clot, carefully stop bleeding and suture the wound after the above treatment. (2) Hemorrhage of superficial dorsal vein of penis, which is caused by the detachment of ligature, all forms hematoma, and the bleeding is stopped after careful ligation of blood vessels by reoperation. Prevention: (1) During the operation, the wounds exposed after the proximal foreskin retracts should be carefully stopped bleeding, especially at the superficial dorsal vein of penis and the frenum, which can be sutured in the shape of "U" to achieve hemostasis while ligating the skin. (2) Apply estrogen 1mg to adult patients after operation, three times a day, to prevent bleeding caused by penile erection at night. Second, infection, mostly caused by edema and urine impregnation, was cured after anti-infection and local dressing change. This operation is a "ⅱ" incision, so strict disinfection and aseptic operation should be carried out to prevent urine immersion and reduce edema after operation. Third, skin necrosis is mostly caused by incorrect application of electrotome. Prevention: The application of electrotome should be avoided as far as possible, or the penis should be wrapped with saline gauze to avoid skin necrosis, and adrenaline should not be added during local anesthesia. Fourth, the scar stenosis of the foreskin incision needs to be circumcised again because the inner plate of the foreskin is removed less and retained for too long, and the scar stenosis is formed after healing. Prevention: The length of the foreskin is appropriate, and the inner plate cannot be kept too long. In addition, an oblique incision with oval cross section is made to form a relatively large foreskin opening. Five, postoperative rubber swelling due to local scar formation affected the lymphatic reflux at the far end to form rubber swelling. When re-operating, an oblique circular incision should be made near the coronary sulcus to remove rubber swelling tissue to the proximal side. Sleeve ring resection should be adopted as far as possible, and the inner plate should not be kept too long to avoid rubber swelling. Six, epidermoid cyst due to the formation of small skin fragments left by pruning, cyst resection. Precautionary operation should be careful to prevent skin fragments from being left behind, and the skin should be neatly stitched to prevent curling in the incision. Seven, the foreskin is too short and the frenum is too short. Patients need reoperation because of limited erection and pain, which affects sexual function. Prevention, resection length is not easy to master, so the operation should be performed by experienced doctors. Many friends often ask me the following questions: Does a small circumcision need so much trouble? Just sew a few stitches casually and then wrap it casually. There is no need to be too elaborate, right? In fact, foreskin surgery is actually a delicate operation with a very high difficulty coefficient. First, the appropriate length of excision, whether the edge is smooth, how to keep the inner and outer plates uniform, how to make the appearance exquisite, how to stop bleeding, how to sew, and how to dress properly? Specific and complicated problems such as when to change dressing and how to prevent incision infection need to be accumulated all the year round, and careful summing up of experience can ensure the best surgical effect. I have been engaged in andrology surgery all the year round, and there are many operations, which are very stressful and very hard. Only I know the ups and downs best. Since I have done this job, I must resist the pressure, work hard, specialize in surgery every day, and concentrate on every little detail! Circumcision is a small operation, because of its special position, which affects the quality of sexual life. It is a high-standard and fine plastic surgery, especially for the comprehensive quality of surgeons. It is not only necessary to know how to do it, but also to do it well, meticulously and beautifully, and to have the advantages of artistry and long-term effect. Without more than 1, cases of surgery and experience, it cannot be accomplished with ease. Especially in the past three years, I have performed more than 1 operations every year. No matter sleeve excision or stapling circumcision, all kinds of complicated situations have been encountered, and the operations have been successful. In my opinion, the quality of operation depends on the accumulation and understanding of daily operation experience, and on the careful operation. Therefore, the difference of operation is not the difficulty, the speed and slowness of operation time, but only the quality.
