Article 1 Students shall enjoy medical insurance benefits for college students from the date when they go through the admission formalities and obtain valid certificates (student ID cards) (September -65438+ February of this semester), and then voluntarily participate in insurance payment every year; Stop enjoying the medical insurance benefits for college students from the date of leaving school, and cancel the school registration according to the regulations on school registration management.
Article 2 Students who go to the school health station for medical treatment must carry campus cards. After the card proves that the students are insured and can enjoy medical security benefits, they will receive Class A drugs 10% and Class B drugs 20%.
Article 3 According to the requirements of the medical security system for college students, our school has set up health stations and implemented the system of on-campus medical treatment and off-campus referral.
Off-campus medical care:
① Outpatient service: The doctor of the health station issues a referral form and visits the designated medical insurance hospital designated by our school. Students pay for medical expenses first, and then go back to school to be reimbursed according to regulations. The general outpatient medical expenses without referral shall be borne by the students themselves.
② Emergency: Students who have high fever, asthma, acute abdomen, upper gastrointestinal bleeding or fracture during the period of stopping medical treatment in the health care station can choose any designated medical insurance hospital for emergency treatment. The medical expenses shall be paid in advance by the students, and shall be reviewed and reimbursed as required when returning to school.
Article 4 Designated hospitals for medical insurance:
① Baoshan Campus Students: Renhe Hospital, Baoshan District (Changjiang West Road 1999)
② Students in Wudong Campus: tongji hospital Branch affiliated to Tongji University (No.50 Chifeng Road)
Fifth students in the weekend, holidays, winter and summer vacations in the off-campus emergency medical expenses according to the provisions of the Medical Insurance Bureau for review and reimbursement.
Article 6 According to the provisions of municipal medical insurance, each person shall pay the corresponding insurance premium every year. The deductible line of outpatient and emergency medical expenses is set in 300 yuan every year, and the part that exceeds the deductible line every year is reviewed and reimbursed according to the hospital level. First-class hospitals are reimbursed 70%, second-class hospitals are reimbursed 60% and third-class hospitals are reimbursed 50%.
Article 7 The time for reimbursement of medical expenses in our school is 25th of each month 10, 13:00- 15:00. In case of weekends and holidays, it will be postponed to working days, and reimbursement will be suspended in winter and summer vacations. The final reimbursement date of off-campus emergency medical expenses this year is March 25th of the following year 15:00, and it will not be reimbursed after the deadline. The reimbursement place is in the health care station, which is reviewed and signed by the students, and then submitted to the finance department, who will enter the student's agricultural bank card according to the reimbursement amount. When reimbursing medical expenses, students must bring their student ID card, referral form, medical record book, medical expense invoice and detailed account sheet.
Article 8 Students are hospitalized:
① Hospitalization in Shanghai: Before leaving the hospital, you must bring a copy of the admission notice, student ID card and ID card to the school health station for review, and issue a hospitalization settlement certificate. When leaving the hospital, submit the voucher to the financial department of the medical hospital, and the medical expenses will be settled directly with the municipal medical insurance center with this voucher. Before leaving the hospital, you don't go to the school health station to issue hospitalization settlement vouchers, and all medical expenses are self-care.
② Hospitalization in different places (only in winter, summer and holidays): you can see a doctor at a designated medical institution in your area, and the expenses will be paid in advance. Within 6 months after discharge, you can register at the medical insurance station with the discharge summary, original invoice, detailed bill and your student ID card, original ID card and its copy, and report to the District Medical Insurance Bureau for review and reimbursement according to relevant regulations.
Ninth students suffering from uremia, malignant tumor, psychosis, hemophilia, aplastic anemia and other serious diseases, according to the serious illness reimbursement system.
Article 10 Students who go to the hospital for medical treatment cannot be reimbursed for the following situations: Chinese medicine decoction, cosmetic surgery, disposable imported medical materials, suicide, self-mutilation, alcoholism, fighting, drug abuse, sexually transmitted diseases, tooth washing, orthodontics, laser myopia, animal bites, vaccination, traffic accidents, medical accidents, unplanned abortion, etc.
