Information needed for medical visa processing

Two 2-inch color photos

2. Original passport, original ID card and copy of household registration book.

3. Appointment certificate

Materials for treatment or recuperation in domestic medical institutions or convalescent institutions in Korea.

4. Hospital diagnosis or doctor's advice.

Proof materials issued by domestic and foreign medical institutions or convalescent institutions that can explain the need for treatment or recuperation.

5. Proof of ability to pay

Proof of ability to pay for treatment, rehabilitation and hospitalization (usually bank deposit certificate)

6. Letter of Credit Guarantee

The immediate family members (same household registration book, or husband-wife relationship) of the business travelers need to provide a letter of guarantee and attach a copy of their ID cards; Or the company provides a letter of guarantee with official seal. Please refer to the template for details.

7. Temporary residence permit

If the travel passport is issued at a different place from the sending place, please provide the original valid temporary residence permit issued six months ago.
