Amblyopia causes their vision to be relatively unclear, so they like nearsighted objects, which will lead to myopia for a long time. But the primary myopia is mostly functional, that is, normal vision can be restored through functional training. If you continue to use your eyes abnormally at this time, including wearing glasses from time to time, reading at close range, reading in the dark, lying on your stomach, reading on your side, etc. And reading books sideways can also lead to other ophthalmic problems, such as strabismus, image fusion obstacles, and it is impossible to establish a three-dimensional sense.
The formation mechanism of myopia is that myopia excites the sympathetic nerve of the eye, relaxes the ciliary muscle, and the lens retracts elastically, so that the light from nearby diverges and projects onto the retina. At this time, the regulation function of ciliary body is great. With the overuse of eyes, the excitability of sympathetic nerve decreases (which should be related to the consumption of neurotransmitters), and the excitability of vagus nerve increases, which leads to the contraction of ciliary muscle and the traction of suspensory ligament of lens. The lens becomes flat, or there is no obvious change, but at this time, the light cannot be projected onto the retina normally, and the retina retreats to receive the image of the object. Moreover, the elasticity of the retina in adolescence is excellent, and the pulling and squeezing effect of the extraocular muscles makes it possible to retreat later and return to the normal position at first. If you use your eyes improperly for a long time, it will lead to organic changes in the retina and form myopia.
In fact, there is another part that plays a great role in the formation of myopia, that is, the organic changes of the cornea. In fact, the cornea is more elastic. According to Chinese medicine, the cornea belongs to wood and is very recoverable. In the process of myopia formation, it should be that after lordosis, it cannot return to its original position normally, leading to myopia formation. The choice of positive and negative adjustment mirrors during ophthalmoscopy should include the causes of myopia caused by corneal diopter theory.

The practice life of ophthalmology is not as simple as imagined. Everyone says, "What can ophthalmology do? It's just drops and medicine. " In fact, this is not correct. Eye drops also have a lot of knowledge, such as eye drops, eye ointment, mydriatic liquid, mydriatic liquid and hormone eye drops. You should pay attention to what you order first and then what you order. There are rules. Checking your eyesight is not as simple as you see. For those patients who can't see within 5 meters, we need to measure the index and light perception, which I didn't know before. Although the process of internship is arduous, the content of our study is constantly enriched and deepened, and our knowledge is constantly consolidated and increased. Summary of Ophthalmology Practice 4 Because we are in two groups together, Mr. Xia Qun divided us into two groups, and Yan Peng, Shi Yan and I were the first to practice in the ward. The ophthalmology ward is located in C09. There are oral plastic surgery, but there are few patients in the oral cavity, and parotidectomy and other operations are also performed. There are no patients in the plastic surgery department, only patients who borrow beds from endocrinology and neurosurgery. Ophthalmology still accounts for most of the beds, about 35. When I first arrived in the ward, there were no doctors in the doctor's office, and most of them were examining patients in the examination room (including checking eyesight, measuring intraocular pressure, slit lamp examination and dressing change). The three of us also squeezed in, but soon there were more people. We also gave the patient some eyesight tests, and the teacher also showed us the slit lamp. Look at the intraocular pressure and basically know how to measure it. Then I went back to the doctor's office and learned that I could go to the eye operating room to see the operation, but only two people could go, so Peng Yan and Shi Yan followed Guo Liping, the deputy chief physician, and I stayed in the office to read. I spent the whole morning in ophthalmology and met some ophthalmologists. Ophthalmology is divided into three groups. Except one group that is responsible for cataract, the remaining directors Dai Hong and Shi Zian are responsible for one group respectively. I mainly follow the group of Harmony (in the director group), mainly because there are many people in their group, Zhao Jingzai is in the director group of Dai, and there is a resident Zhu Zhu (nickname). I don't know why many ophthalmologists have nicknames. Besides Zhu Zhu, there are Guang Guang, Gundam and Zhao Da. I still can't compare with some. In addition, Wang Zheng and Chen Tong are considered as Corey's pistachios, especially when Wang Zheng speaks with some strange voices from time to time, which makes people laugh. In addition to these doctors, Lu Guang and Zhen Xiao belong to Director Dai, and there is a new graduate student, Yang Xu, who is a xx clinical student, but her tutor is Director Dai. There is also an attending doctor, Lu Yingyi, who is responsible for receiving patients. When I first met her, I thought she was a few years older than me, and she was the chief resident (internal medicine is the chief resident). Later, I learned that she went to college in 1994 and just gave birth to a baby, but she still looks very young. The time in the ward was really boring. I read the medical records and books, and there is nothing else to do. I went to the operating room twice, and after seeing the macular edema ball, I took medicine and had cataract surgery for a short time. Wang Zheng, a pharmacist in particular, doesn't like to accept it. This kind of surgery clinic can be done, but in order to increase the turnover rate and economic benefits, it is still accepted. But all of them were hospitalized for laboratory tests today and discharged the next day, but everyone was unwilling to accept the medical records. Cataract surgery is a very common disease and can be done in half an hour. The cost is fixed. The only difference is the price of intraocular lens. More than 30 kinds of crystals from 7 or 8 companies have won the bid through bidding, with costs ranging from 200 to 6,000 and different styles. The only regret is that I didn't take a fancy to enucleation and artificial eye implantation. The sclera was bought from a colleague's eye bank, and the patient was pushed into the operating room, but his blood pressure was 200, and he was blown down again. He didn't take a fancy to it and cried. Out of the ward, he went to the auxiliary examination room and saw all kinds of advanced ophthalmology equipment, such as FFA, ICG, OCT, dynamic visual acuity, GDX, krypton laser (therapeutic sugar net), which looked like flies after taking them. But it has little to do with the disease. However, I was in the clinic when many patients were re-examined, so I could learn more. The luckiest thing is to see a patient treated with PDT (that is, the one who took medicine 16000) (or the wife of a doctor who studied endocrinology during her internal medicine internship).
Finally, when I went to the clinic, I was lucky enough to follow the director and deputy director Yu for a long time. Director Shi Zian is the deputy director of ophthalmology. She has too many patients. In addition to the number hanged that day, there are reservation numbers and plus signs. Anyway, there are many people. I followed the director of the department to see a doctor, helped dilate the pupils and prescribed medicine for the patients. So I haven't finished seeing the patient from 8 am to 65438+ 02: 30 noon. The director didn't even take a sip of water, let alone have a rest. I came this afternoon. In the afternoon, I followed Deputy Director Yu. The first thing she said to me was to ask me to help her get her seal back. I was busy making a report at noon and left my seal in the outpatient building. In the afternoon, she doesn't have many patients. We chatted for a while and learned that she went to the North Building to inspect an African leader in the morning.
On Friday, I was in the treatment room. Because Beijing Hospital celebrated the Double Ninth Festival, registration, intraocular pressure measurement and blood sugar measurement were free that morning (usually these items were at least 10 yuan, and many old experts came out for free consultation), and a large part of the workload in the treatment room was to measure intraocular pressure, so I kept measuring intraocular pressure for people all morning, and many people waited in line for the test and didn't know what intraocular pressure was for, so China people took advantage. ! In short, I have measured about 50 or 60 people, which is estimated to catch up with twice the usual workload, while the doctors outside are more tired and the patients are all in long queues. It seems impossible to eat on time at noon. It is said that the people who take the number downstairs in the lobby are crazy, there are too many people. However, I was still busy in the treatment room and chatted with head nurse Liu for a while. BTW, it seems that someone in Class 4 is determined to follow Director Dai. Every time Director Dai went out of the clinic, he skipped classes from the People's Hospital and came to "be caring and attentive". In Shi Yan's words, he was so poor that she didn't like him when chatting with the head nurse. But how can I put it? Everyone has their own ideas about the future.
In short, I have gained a lot in ophthalmology these days, at least I can measure intraocular pressure and see some very simple diseases, but ophthalmology is indeed a very fine department, so it is no wonder that there is a saying of golden ophthalmology.
