Some time ago, a news in Wuhan once again sensationalized the society.

The young man was ill and had a fever, so he went to see a doctor. After several twists and turns, he was finally found to be HIV-positive in Zhongnan Hospital of Wuda University, and it has reached the late stage of AIDS. The doctor analyzed that the man may spread and infect his sexual partner without knowing it.

Expert: It is very common to find that the diagnosis is delayed late.

Events in the news are not unique. There are many cases of delayed diagnosis of HIV in China, and even many cases were diagnosed only in the late stage of AIDS.

Most infected people have symptoms such as diarrhea, fever, headache and rash before going to the hospital, but many non-specialist hospitals only deal with the symptoms, often ignoring the possibility of HIV infection, which may have been delayed for a long time.

According to the statistics of Peking Union Medical College Hospital, from the first typical symptom of AIDS to the diagnosis of AIDS, most people went to more than three hospitals and five doctors, and went through a course of seeking medical treatment for about two months. The overall detection period of AIDS patients in China is relatively late, and the CD4+T lymphocyte count of many patients is lower than 50 /mm3 when they are found, which leads to two important problems: difficulty in treatment before diagnosis and susceptibility to infection.

Not only in China, but also in many countries.

According to the data of French Public Health Bureau, from 2065,438+07 to 2065,438+08, more than 65,438+0/4 new infections were diagnosed in the late stage.

In Britain, when 40% of newly infected people are diagnosed with AIDS, the CD4 count is lower than 350 /mm? It means that 40% of infected people have just entered the AIDS phase after being diagnosed with infection, and 20% of new infected people have CD4 as low as 200 /mm? .

Why is there a late discovery?

The late detection of HIV-infected people and AIDS patients reported by medical institutions in China from October to July, 2009 was analyzed, and it was pointed out that the late detection accounted for 68.4%. In this study, 293 187 HIV/AIDS patients who were diagnosed with HIV infection were tested for CD4 count within 9 1 day. From 2009 to 20 17, the proportion of newly reported adult AIDS in medical institutions in China remained at a high level.

The analysis points out that the late detection is mainly related to male, Han nationality, large age group, low education level, married spouse, other patients' testing and permanent population.

Professor Li Taisheng of Peking Union Medical College Hospital believes that there are two important reasons:

First, patients lack knowledge about AIDS, and they don't know what high-risk behaviors are and what situations need to be screened for AIDS.

Second, some medical staff do not know enough about AIDS knowledge, such as the six common symptoms of AIDS, such as young patients who suddenly have repeated lung infections and meningitis and ignore the possibility of AIDS.

In addition, fear of HIV testing, denial of HIV infection and fear of discrimination are also important reasons for delaying diagnosis.

Early detection and early treatment require the participation of the whole society.

At present, there are still 30% infected people in China who have not been found. It is not excluded that some people are already in the late stage of AIDS when they are found. In addition to delaying treatment, it is more likely to cause invisible transmission. Therefore, it is particularly important to improve the detection rate and reduce the diagnosis delay.

Medical institutions are the main way to find AIDS cases, especially in primary medical institutions, it is easy to ignore the flu-like symptoms in the early stage of AIDS infection. At the social medical level, we should vigorously improve the early detection ability of AIDS in medical institutions in China, implement the testing and consulting strategies provided by medical staff in medical institutions, and promote the early detection of high-risk behavior groups.

At the personal level, in addition to the hospital's disease control and blood test, you can also choose self-test that can be carried out at home, such as saliva test and blood test. Improving the early diagnosis of HIV infection requires the joint efforts of society, the public and medical staff.
