Handling process and matters needing attention of application for door-related diseases
1. Fill in the application form: the insured person needs to bring his social security card or ID card to the medical insurance workstation or designated hospital to receive the Confirmation Form for the Application of Door-regulated Diseases in duplicate and fill in the relevant information. The application form needs to be filled in and signed by the attending doctor, and it needs to be signed by the department director for confirmation.

2. Preparation of application materials: prepare relevant medical records, examination reports and other materials according to the applied diseases.

3. Hospital confirmation: submit the completed application form and prepared materials to the medical insurance department of the hospital for review. After the hospital confirms that it is correct, affix the official seal of the hospital on the application form.

5. Medical insurance approval: submit the application form with the official seal of the hospital and related materials to the medical insurance agency for approval.

6. Physical examination: After approval, the insured shall go to the designated hospital for physical examination according to the notification requirements.

7. Expert appraisal: After the physical examination, experts will conduct appraisal and record the results. Get the Door Regulation Certificate: After the appraisal result is fed back to the Municipal Medical Insurance Office, the insured can get the Door Regulation Certificate at the medical insurance workstation.

8. Precautions: The time, place and materials required for applying for on-site illness may vary according to different regions and diseases. It is suggested that the insured person consult the local medical insurance department for specific requirements in advance.
