Desmopressin, a hemostatic agent
Desmopressin is used to prevent and treat bleeding in patients with VWD or hemophilia A, whose condition is moderate and mild, accompanied by further primary hemostasis dysfunction, such as uremia, cirrhosis or aspirin-related bleeding. 1- deamino -8-D- arginine vasopressin (DDAVP) is an analogue of arginine vasopressin, mainly V2 agonist. The regulation of V 1 receptor is vasoconstriction. Although the recommended dosage of DDAVP is 0.3ug/Kg, it will increase the level of FVIII, VWF (especially high molecular weight polymers) and tissue plasminogen catalyst by 2-20 times. DDAVP is not always effective in reducing perioperative bleeding, and it has not been found to have a satisfactory effect in other clinical consequences. What's more, it is related to the 2-4 times increase in the risk of myocardial infarction. Meta-analysis of 12 randomized controlled trials using DDAVP in cardiac surgery showed that the incidence of MI in DDVAP treatment group was 4.4%, while that in placebo treatment group was 1.6%(OR, 2.07; 95%CI,0.74-5.85; P=0. 19).Cattaneo and Mannucci collected data from a placebo-controlled double-blind experiment to evaluate the efficacy of hemostatic DDVAP in cardiac, vascular and plastic surgery. Total thrombotic events (such as myocardial infarction, cardiac arrest, arterial thrombosis and venous embolism) were reported by 3.4% in DDAVP group and 2.7% in placebo group, respectively (P = 0.38). The authors collected data of 15 hearts, 2 vascular operations and *** 17 randomized controlled trials, which clearly informed the existence of embolic events. The incidence of MI in DDAVP treatment group was 3.9% (28 cases in 716), while that in placebo group was 3.1%(20 cases in 646), and the relative ratio (OR) was 1.25(95%CI, 0.72-). p=0.43)。 In plastic surgery, three of the five randomized controlled trials to evaluate DVT were reported. There were 4 cases of DVT and/OR pulmonary embolism in DDAVP control group (3.7%), 5 cases of DVT(4.3%) in placebo group, and OR was 0.82(95%CI was 0.24-2.78; p=0.75)。
