4. Preliminary review, filling and issuance:

For those with obvious disability characteristics and easy identification of disability categories and levels according to disability standards, the county-level Disabled Persons' Federation directly fills in the evaluation form and the county-level (including county-level) or above hospitals or specialized medical services designated by the county-level Disabled Persons' Federation If the disability assessment result made by the institution meets the disability standard, the relevant information of the disability certificate should be filled in and printed according to the disability assessment result, and submitted together with the application form, assessment form and other materials in triplicate to the municipal Disabled Persons' Federation for review and approval.
The chairman of the county-level Disabled Persons' Federation must sign the person who issued the disability certificate and stamp the official seal of the issuing authority in the issuing authority column.
5. Review, approval, and filing:

Approval will be given to those who meet the disability standards, are accepted by the county-level Disabled Persons' Federation, have correct preliminary opinions and comply with procedural regulations. 6. Issuance and filing:
